Maybe the feedback you've had is meaning that how you've structured the research proposal rather than the research idea itself needs revision. Have you/could you ask an academic you currently have access to, rather than one you're applying to work with, to read the proposal through just to see if they can give you any advice on whether in general terms it follows the format they would expect?

Agree with above that there may be some problems of applicability, e.g. with the question about driving for those who don't drive.

Congratulations Pineapple, you've certainly earned it.

May not be relevant, but is there no-one else you could ask for the second reference e.g. who knew you from an undergrad degree? Since you have one reference related to your MA it wouldn't be likely to appear that you are hiding bad news if you get a second reference from another source.

If all else fails, would put in the application and note that your second referee would prefer to send a reference on request once your MA marks arrive at Easter. Then at least you've done all you can.

This is extremely minor and may be too complicated to be worth bothering with but when spam has been deleted would it be possible to revert the latest post listing to the last relevant post (e.g. the working at home thread) otherwise I look for new posts and they aren't there and I'm easily confused :(.

Could your supervisor just mean that in general terms books will have fewer words to the page than the standard A4 print out due to smaller pages and, probably, larger margins? If she's got a particular format in mind I think you'll need to ask her for specifics otherwise searching on 'book page layout' or some such yields various more or less standard templates for books or looking at relevant publisher's guidelines from their websites may help.

Just read your second post,if this is in relation to the format of the thesis itself, the university should provide guidelines somewhere on the website or postgrad handbook. I think the norm is to print on A4 though the margin on the binding side has to be greater than usual

If you're willing/able to use a programmming language a bit R might be an alternative www.r-project.org/ which is a free download. I haven't looked at it in any great detail but have been told by others that it's useful and not monstrously difficult to learn.
